This form is the fastest and easiest way to book with me, with everything I need in one place. Please note all new clients are required to submit a small deposit before I will schedule an appointment. At this time I do not accept any other forms of payment for deposits.
Due to the time it takes to screen, I am nearly never available for same-day appointments with new friends. If you are giving references, please be sure to let them know beforehand. I am happy to accept reputable known references. Please also send a current selfie for my reference.
Personal phone number is for verification purposes only. I do not accept VOIP or app numbers for this reason, though I am happy to contact you there if you prefer. Texting for contact won’t be done unless specifically requested.
